1. Sorry dude, no sims in multiplayer. (bums me out too)
2. Depends what difficulty your on. On Normal, it probably won't take you more than a night or 2.
3. You start behind your co-op buddy, after he has cleared all of the enemies from the area and stopped shooting. (it sometimes takes a while)
