I finish the main quest with my character at level 59. I have receive the blood ring that the lady of Azura give me. But when i return to Lord Vivec, he say to me that there a lots to do now. My fame rating 49. I want it go up 100...maybe there are secret quest that you need to perform to take you fame going up....
At 59 there should still be plenty to do. For me, I couldn't complete the Redoran, but I've completed everything else except for the Imperial Shrine or whatever it's called.
After that, I guess all that's really left are the random side-quests from when you meet people. It kind of seems like Vivec would have things for you to do from the way he talks, but I haven't found it.
