Here they are folks.-

1) Hold L+R triggers and push YYYY at the character select screen to get everyone's 5th outfit.

2) Create a new game and use Golf_Gone_Wild as the name. That'll unlock EVERYTHING (except the 5th costumes).

Was aware of both of them but I'm using this for the fifth outfit:

While holding left trigger:

Y+Y+white+Y+black+Y

I KNOW this works. Are you sure that yours does too?
